About

Divers Alert Network (DAN) has maintained a diving incident database since 1989. Originally it was limited to scuba diving incidents. This online reporting system is designed to include breath-hold diving and rebreather incidents separately from scuba incidents.

Data

Persons reporting cases are not required to provide their name and contact information. The information is requested to facilitate follow up if warranted (and if the person grants permission for contact to be made - asked as one of the final questions). The results of our data collection and analyses may be made public for the benefit of the community. Data are released only in anonymous form. No individual shall be identified in the reviews of cases presented verbally or published.

All information obtained during the study, including the respondent's identity, will be kept confidential to the full extent allowed by law.

Instructions
  • The online incident report form is divided into steps.
  • Although the time to complete each step will vary depending on the detail and complexity of the case, each step must be completed within 18 minutes or the data entered will be deleted. A "Time to Complete Page" timer on each page will show you how much time you have left to complete the step.
  • Each step will be saved automatically when you click on the submit or continue buttons. Then you will automatically progress to the next step.
  • You will be able to review saved steps to make changes if necessary. However, once you have clicked the "Done - Submit Incident Report" button on the final step. Then, you will no longer be able to review or edit the incident..
